REQUIRED FORMS:

Incoming freshman, junior, and new-to-swim athletes: Current physical + medical history form 

Incoming sophomore and senior athletes (not new-to-swim): Current medical history form 

ALL athletes: Additional online forms requiring digital signatures 

An Asthma Action Plan completed by a physician is required for athletes with asthma.
